Acknowledgement of Services and Assumption of Risk Agreement

(For Pain Quiz video responses & general communication with persons that have not had a live video assessment)

ACKNOWLEDGEMENT OF SERVICES

I agree that communication between myself and Brandon Launstein before we've had a live video assessment is meant as general discussion and education about musculoskeletal health and does not constitute professional medical advice, diagnosis, or treatment by Brandon Launstein. Any communication between myself and Brandon Launstein before a live video assessment is for general education only and is in no way intended to create a provider-patient relationship as defined by state or federal law. My interactions with Brandon Launstein are not intended to take the place of my relationship with my regular health care practitioners or primary care physician. A Pain Quiz video response does not replace professional medical advice.

ASSUMPTION OF RISK

I agree that if I engage in any physical exercise or activity described by Brandon Launstein, I do so at my own risk and assume the risk of any and all injury, damage, and/or loss I may suffer.  Any exercise has potential to cause injury or pain if it is incorrectly done or is not the right exercise for an individual’s medical or physical problems. I agree that it is my responsibility to determine whether I am safe to perform an exercise or activity. If an exercise or activity requires the assistance of a family member or helper (collectively “Helpers”), I am  accepting the risk of the actions of my Helpers. I should conduct a live video or in-person assessment with a physical therapist or other medical provider for specific medical advice.

I expressly agree that the information on this page which includes the acknowledgement of services and assumption of risk agreement is intended to be as broad and inclusive as permitted by the law. I acknowledge that I have carefully read this document and am aware and agree that by engaging in communication with Brandon Launstein and/or submitting a Pain Quiz, I am forfeiting my right to bring legal action or assert a claim against Brandon Launstein to the fullest extent permitted by law in my state of residence. I have read this document and agree that no oral representations, statements, or inducement apart from the foregoing agreement have been made.
